Welcome to the website of Revive Physical Therapy, LLC (“Revive Physical Therapy,” “we,” “us,” or “our”). By accessing or using our website, located at www.reviveptks.com (the “Site”), or scheduling services through our integrated booking platform, you agree to comply with and be bound by the following Terms and Conditions (the “Terms”). Please read them carefully. If you do not agree to these Terms, you should not use this Site.
1. No Medical Advice or Patient-Provider Relationship
The text, images, graphics, blog posts, FAQs, and other materials contained on this Site are for informational and educational purposes only. The content on this Site is not intended to be a substitute for professional medical advice, diagnosis, or treatment. * Accessing this Site, submitting a contact form, requesting a 15-minute Discovery Call, or reading our educational content does not create a physical therapist-patient relationship between you and any therapist at Revive Physical Therapy.
A clinical relationship is only established after you have completed an initial formal evaluation and signed our internal clinic-specific intake and informed consent documentation.
Always seek the advice of your physical therapist, physician, or other qualified healthcare provider with any questions you may have regarding a medical condition. Never disregard professional medical advice or delay seeking it because of something you have read on this Site. If you think you may have a medical emergency, call 911 or your doctor immediately.
2. Age Restrictions
Our services and website content are intended exclusively for adults. By using this Site or requesting services, you represent and warrant that you are at least 18 years of age. Revive Physical Therapy, LLC does not knowingly evaluate, treat, or collect information from individuals under the age of 18.
3. Services and Practice Model
Revive Physical Therapy operates under a cash-based (out-of-network) model. We do not contract with or bill private commercial insurance providers or Medicare.
Payments: Payment is due in full at the time of service. We accept credit/debit cards, checks, Health Savings Accounts (HSA), and Flexible Spending Accounts (FSA).
Cancellation Policy: We enforce a strict 24-hour cancellation and rescheduling policy. If you fail to provide at least 24 hours' notice before your scheduled evaluation or treatment session, or if you miss an appointment, you will be subject to a cancellation charge at the sole discretion of Revive Physical Therapy.
4. Online Booking and Third-Party Links
Our Site integrates with a third-party booking platform (Jane App) to facilitate scheduling. Your use of the booking platform is also governed by Jane App’s terms and privacy protocols. Our Site may also contain links to other third-party websites. We are not responsible for the content, privacy policies, or practices of any third parties.
5. Intellectual Property
Unless otherwise stated, all content on this Site—including articles, logos, graphics, layout, and text—is the intellectual property of Revive Physical Therapy, LLC and is protected by copyright and trademark laws. You may read, print, or download material for your own personal, non-commercial use, provided you do not alter the content or remove any copyright notices.
6. Limitation of Liability
Revive Physical Therapy, LLC, its owner, and employees shall not be liable for any direct, indirect, incidental, or consequential damages resulting from your use of, or inability to use, this Site or the information contained herein. We do not guarantee that the Site will be completely error-free or uninterrupted.
7. Governing Law
These Terms are governed by and construed in accordance with the laws of the State of Kansas, without regard to its conflict of law principles. Any legal action arising out of your use of this Site must be brought in a court of competent jurisdiction in Johnson County, Kansas.
